If you change your name and address or telephone number, please inform us as soon as possible. See our change of details page for further information. If you move out of the practice area, you will be required to reregister with a more local practice.
How to Register as a Patient
If you live in our practice catchment area you can join our list. If you are registering your new baby please next section.
To register please visit the surgery with the following completed forms and information:
Consent Form for someone else to access a patients’ medical record (If applicable)
Proof of Identification (ID) (for example a passport, driving licence or medical card [you may be asked for two forms of Identification])
Proof of Address (for example utility bill or bank or credit card statement from the last four months, current tenancy agreement, government agency letter, higher education letter or a letter from a recognised employer).
The General Medical Services (GMS1) and New Patient Forms are also available by visiting the surgery and ask reception for a paper copy.
How to Register your New Baby
You need to register your new baby at the practice as soon as possible. Therefore please dowload and complete a General Medical Services (GMS1) Registration Form (also available from reception) and send it to your surgery. Please ensure you provide your baby’s NHS Number which will be detailed in their Red Book. Once we have received the form we will register your baby as a patient at this Practice.
The Practice will send you an appointment for a mother and baby post-natal check. This is a 20-minute appointment, 10 minutes for the baby check and 10 minutes for the post-natal. Your baby’s course of routine immunisations will start at 8 weeks and will be given at this appointment. Please remember to bring the red book. If you cannot make this appointment, you must notify reception as soon as possible. Please be advised that if you do need to cancel, the next available appointment may be anything up to 4 weeks after that date.
Safe Surgeries
Everyone in the UK has the right to free care from a GP. If you don’t have proof of ID or address, you can still register. Immigration status or nationality don’t matter – reception won’t ask for immigration documents and won’t share your information with the Home Office unless serious crime is involved. In partnership with Doctors of the World UK, we will ensure that our practice offers a welcoming space for everyone who seeks to use our services.
